Same as always; where does the current go, and what does it do when it gets
there? Of course, for ESD, it could be radiated, but the question is the
same. If one can't keep it out, he must find and harden vulnerable
devices/circuitry. 

I've had some luck injecting short (and much lower energy) pulses directly
to identify what needs to be done. And also looking hard at the schematics
and layout. It's amazing the errors that can creep in. But for what it is
worth, many I/O ports can tolerate a few bypass capacitors or clamping
diodes.
