For reference, the medical device EMC Standard, IEC 60601-1-2:2007 3rd ed.
states the following for -4-4 compliance:

 

6.2.4 Electrical fast transients and bursts

 

ME (Medical Electrical) EQUIPMENT and ME SYSTEMS shall comply with the
requirements of 6.2.1.10 at an IMMUNITY

TEST LEVEL of * 2 kV for a.c. and d.c. power lines and * 1 kV for signal and
interconnecting

Cables [over 3 m in length].  

 

Upon further reading of the 60092-504 requirement you provided, I wonder if
they are trying to describe two different ways that some people might call out
the power entry means.  Since -4-4 is a common mode test, with the 33 nF caps
directly coupling to each mains conductor and the protective earth conductor
in the CDN.  I don’t think that -504 is asking you to test line-to-line
differentially.  I’m not sure how you would even test Burst/surge
differentially on a power port.

We are preparing a burst test (fast transient – IEC61000-4-4). 

The specification (IEC60092-504) states: Amplitude 2kV line / line on power
port. 

 

Question: Is that a typing error? I have never heard of line-to-line burst
testing.
