I think it's hard to adequately cover all the possible failure modes that can occur with an MOV. 

I have seen some implementations include a series fuse that would help for certain failure modes.  I also recently saw an advertisement for a device that packages an MOV with a series fuse that is thermally triggered. 

My own approach to MOVs has been to avoid using them if possible, but I understand that there may be certain applications where MOVs are the best match for the application.
