Evening A few weeks ago I went to a UK Electrical Safety First (ESF) Seminar, and, IIRC, this subject was covered in a presentation where it was said that the main point of failure was the Primary-to-Secondary insulation – most did not even meet the dielectric strength requirements for Basic Insulation L.
