Jon, I've tried to use combination drill/taps with my Tapmatic tapping head with similar breaking results. The problem seems to be that the tapping head allows for sideways float to that the tap will self align with an existing hole. That works great when you are manually positioning a part on a drill press. It did not work well with the drill/tap on the same drill press.
For now, I just drill all the holes and then make a separate pass with a tap.
