Another idea is: You could have a sensor read your A channel pulses. But, you could also send the its pulses to a 74HC4020 or 74HC4040 binary counter IC to count the pulses and get your index pulse. Depending on which output pin you choose on the IC you could get a single pulse every 2, 4, 8, 16, 32, 64, 128, 256, etc pulses. That could simplify the pulse generating plate and the placement of the sensors a little.
