Sounds to me like a Siemens Machinetool Spindle motor.
But the speed-range is a bit wide for constant torque. Do you need the full
torque at 4000rpm?
Might it be with a 2speed (or 3speed) gearbox?

There again: I live in South Africa, so not too much help I guess.
