19,000 lbs is not a very heavy machine for most riggers. 30-40,000 lbs gets more interesting. :-)
You should be able to find a rigger local to the machine to load it, a trucker to move it, and a rigger at the other end to unload and install/position it. Chances are they will all be different companies due to the distance. A reasonable price would be $750 to load, $750 to unload, and a few bucks per mile to move it if they can get another load on the trailer. That would include 2-3 people doing the loading and unloading. If they have to move the machine a distance on skates or via forklift.. that adds to the cost. Time costs money.. Step deck trailers are cheaper than double drops (with a well), they are more common, and can usually carry a second machine cutting transportation costs. The trucker should be responsible for tie downs on the trailer and tarping (if desired). Sometimes it is a good idea to be present to advise on tie down locations so they don't tweak the machine via the tie downs. You might want to shrink wrap the machine. Flapping tarps can tear up machines. I'm in Indiana and can give you some Indiana company names if you would like..
