it seems that the add/remove comment is thought for single line comments,
which html does not support (comments in html are always in block format),
check in C or any other highlighter which supports single line comments and
youll see it working
true that it could be disabled in languages which doesnt support it, but
we cant know how that can be done internally in pspad or the difficulty
attached to that process... either way, this does not mean that it can be
corrected/added in future, so if you think it is working wrong or not
clear enough, instead of writing here the post, you could write in into
the bugs section, and if it were not a bug, post it as a feature request,
so it can be read by the people making the program, the help files and the
translations, all at once :)
anyways, dont get mad, as far as i've read in the forums, we all know
that the help file is outdated, and there ARE people working in an updated
one, so posting your issues with the configuration maybe is a good thing to
help the people making those documents
and lastly, you are posting here so you know this forums, if it is not in
help and you cant make it run, ask!! and ye shall receive!! :-D

about the "local help file" i suppose it is related to a help file for
some particular highlighter, it can be set in menu settings/highlighter
settings, choose the highlighter in the list, click the second tab
("specification"), there you can set the help file, clicking the button
with the 3 points at the left of the 2nd fields
after setting it, it should open when you are working in that highlighter,
under the menu help/help for language
