We once had an issue where our users could connect to our VPN which uses RADIUS auth but occassionally some couldn't.
The cause was one of the two IAS (RADIUS) servers had a policy config typo but as this was a secondary server it was only used sometimes which is why it appeared only intermittently. Many of our other services that use RADIUS can only use 1 RADIUS server so were unaffected. Maybe you have a similar issue.
