I believe the privacy / cookie controls might be best served by
something like uMatrix? However, that is a lot more than just cookie
controls.

I suppose you're right. I might speculate that Mozilla doesn't think
there is much demand, or doesn't think the functionality can be
reliably implemented. Otherwise you would surely just take the code
of an approved add-on and merge it into mainline.
