I was trying "Scenario 4: Install an add-on outside of the Firefox directory, 
but have it still available in Firefox. The user can disable the add-on."

I have this working, the xpi file drops into the "/Library/Application 
Support/Mozilla/Extensions/{Firefox GUID}"

And I've had to modify extensions.autoDisableScopes

Is that the best Firefox can currently do?
How did you disable the about:addons page?
