I am afraid I have to disagree with you. Job titles are separate from
degrees earned a academic institutions and forms used to identify which of
several identically named people you are referring to. In common and proper
English usage, a job title is never placed following a name as a part of the
name, unlike the Ph.D or Jr. suffixes. In this case, Entourage and Palm are
correct and match proper usage of the fields. (Though it could be argued
that Palm should have prefix and suffix fields.)

Note that you also included the form of address Esquire - this is really
separate from all of the above.
