I didn't know it at the time, but I had a serious virus which was causing
lots of crashing, so that I couldn't even start up with extensions off. (I
had turned Norton AntiVirus off at some earlier OS upgrade when it was
wasn't working right, and never turned it back on. DON'T do that!) Booting
from the CD and doing a clean install was the only way to get back. then i
did a full merge, so the virus came back and I had to do it again.

Use TomeViewer on the OS 9.1 Update folder. (If you never did so, you can
drag the unified disk image of it made from all the downloaded disk images
and drag it to your hard disk from the desktop to copy it. If you have to,
you can repeat the "making single disk image" by clicking on the Mac OS 9.1
Update.smi image if you still have all 15.) Navigate to the Installation
Tome in the Software Installers subfolder, and expand "Mac OS 9.1 All" and
"Mac OS 9.1 Base" to your desktop or hard disk. Open CC, go to Sets menu
-->Edit Sets. Click Import, and import these two sets. Then delete the
9.0.45 base and 9.0.4 All sets from the same location. Quit CC and the next
time you restart you'll have the two new sets available.
