that's not my experience. 

I've checked that option many times and watched the extension get added.
Maybe the reason your friend doesn't see the extension is that the
extensions normally don't show in the windoze environment.

Was he able to open the document the way he received it?

I'll try that. If true, I wonder if that's also true of the other settings
in that dialog box, like which encoding is used? But one would think that as
settings become unavailable or irrelevant, they'd be grayed out.
