I don't know what is causing it, but have narrowed it down a little. When this happens, the spell checking is only happening for the subject line (that is why you get the message that spelling check is complete even though you still see errors in the body of the message). In these circumstances it is not even trying to check the body. One workaround that often works, but not always, is to highlight the entire body before selecting the spell check option under tools.
