Forward should do that automatically. In fact, I don't know of any setting in Entourage that will change the behavior for a forwarded message.
For Reply, if you go to 'Mail & News Preferences', on the 'Reply & Forward' tab, you can select the 'Prefix my replies with short header-style attribution' in the 'Attribution' section. That will give you a message like this one.
