It's possible that the file has three adjacent plus signs, causing the modem to return to command mode, when the file is encoded for transmission in the email.
The Hayes patent required that the three plus signs be separated from other information by a pause in transmission (to prevent a random string of plus signs from dropping the connection), but some vendors, in order to avoid paying royalties on the patent, implemented the feature by simply dropping into command mode whenever the data stream contained three plus signs in a row.
