OS X is still a new OS. Every "point" update - 10.2.1, 10.2.2, 10.2.3, 10.2.4 - is primarily a bug fixer and sometimes performance improver. Keep updating every time a new one comes out, or wait a week first to make sure there are no owls of pain, if you prefer. There are no howls of pain with 10.2.4.
You don't have to download the combo. You can just run Software Update to update. If you already have 10.2.3 installed, you will not get the 76 MB combo download, you'll get a much slimmer incremental update. But if you have other machines or want to be "ready" in the case of a clean system re-install (planning one of those?) you would choose the combo instead and save to desktop. I don't think this is necessary myself. If you should ever need to do a clean system reinstall, you can do the combo update at that time.
