I'm not familiar with 'my label designer', but if you go to the tools menu there is an option there to print labels, and many standard label formats are included - virtually all the Avery label sizes.
Also, you can download Avery templates from their web site for many applications, including word. Then, you need to set up a data merge document specifying the address book as the data source. You can set up the template to print whatever you want, wherever you want exactly as you can with a normal document. There's plenty of good advice in the help files, just search under 'data merge'. As to exporting the address book, you can do this from entourage most easily. Just select 'export contacts' from the file menu. A tab delimited file is produced by default. This could then be opened in either word (as a table) or in excel.
