Do you have Excel? Or AppleWorks? You could try exporting from Address Book
into a tab-delimited file, and then importing into a spreadsheet. The
spreadsheet would allow you to move data around fairly easily, rearrange
columns, etc. You could then produce a tab-delimited file with all the data
in consistent fields that you could match up with Entourage's fields during
import.

I can't guarantee this will work, but it's all I can think of. Of course,
Paul's scripts (which he mentioned in connection to the calendar data) would
be a great help with this as well.
