Since the database is considered one file, if a single change is make the
entire file will show as new and need to be backed up. This has been a point
of contention with many Entourage users. Personally, I just duplicate my
identity so this isn't a problem for me. I believe Paul uses Retrospect to
backup and can probably shed more light on his experiences.

A little background on Notifications should shed some light on the problem.
Notifications is a MS applications and runs in the background unless you
turn if off in the application or quit the daemon. See ways to quit here:

The Microsoft Database daemon allows Notifications to work even if all MS
applications are closed. The daemon is actually used all the time to access
the Database. Even Word uses it. This is why you need to Quit Notifications
and/or the Microsoft Database daemon when you install MS updates or run a
backup application.
