Turns out the Proofing Tools folder was missing - presumed to have been left out during the installation. All the same, it would be nice with a more intelligent error message or warning in the Preferences window.

I've tried dumping the Entourage Preferences file in ~/Library/Preferences/Microsoft/ as that's the file that is updated on my own Mac when I change spelling prefs, but no joy. Any suggestions?
