Entourage sure feels and operates like I expect it to do.
Try trashing:
/Library/Caches/com.apple.LaunchServices.6B.csstore
Then launch Entourage and reset Notifications. Log out/in and see if it is
listed.
Did not help for me. What I did see, when I had Entourage set to work off line, Database daemon was not active in Activity Monitor as well as System Preferences->accounts startup items.
