Not sure what it is you have a problem with here... If I right-click on the server icon in the folder pane, in the contextual menu is an item 'New Folder'. This will make a folder at the root level - that is, on the same hierarchical level as the inbox. It will be listed lower down in the listing than the inbox (i.e. Non-alphabetically), because the folders from 'Inbox' to 'Junk eMail' are special and will always be listed first.
However, if I select the Inbox (or any other existing folder) and right-click, I get a contextual menu item called 'New Subfolder' that will create a new folder as the child of the Inbox (or other folder). Note that some IMAP server configurations only allow folders to contain other folders OR messages, not both.
