Most common reason is that there is a rule higher up in the list that is
affecting this mail. Go through the list of rules and carefully examine the
criteria against each item to see if it will catch the friends that are
missed.

Second possibility - have you got more than one category called 'friends'?
You can't make identically named categories through the UI, but there are
ways this could happen. Perhaps the ones being missed are in another
category with the same (or a similar) name. Rules work on category ID, so it
is possible that items with the same category name are not caught by the
rule.

Yes.

Look at custom arrangements.

Under the view menu, select 'arrange by' and 'Edit Custom Arrangements'

Now you can set up a new arrangement with group items by 'message status',
arrange groups by 'message status' (not sure whether you want that ascending
order or descending to put unread messages at the top - try it and see),
then sort within groups by 'sent'.
