That is a huge database and could very well at least be contributing to the problem. One way to check that out would be for the user to create a new Entourage identity and try typing and sending a message from that new identity. Do the delays occur in the new identity? If not, the size of the database is surely a part of the problem.
Delays in typing usually mean some kind of activity is going on in the background, such as auto-deletion of folders, etc. Another possibility is having very large numbers of messages in folders and doing something that causes access to those folders. The delays when sending could be caused by rules that run on outgoing messages, particularly if they invoke AppleScripts.
