Apparently I was missing the correct mdimporter plugin. The correct plugin is named "Microsoft Entourage.mdimporter". I don't know where the other plugin came from.
I recently wiped my machine and reinstalled everything because of Apple's latest security update. I did not start from scratch with Office, however. I dragged a fully patched Microsoft Office 2004 folder from a backup to my Applications folder. Apparently Microsoft's self-healing installer doesn't replace the mdimporter plugin for Entourage. Re-running the 11.2.3 updater did replace it, though, and I'm back up and working again. If I'd only kept trying for a little longer, I wouldn't have bother all of you with this.
