The advanced rebuild worked. I rebooted and all was okay. Today, however, entourage is acting like a brand new program. The startup assistant launched. What do I do know?

I would make a duplicate of your Identity then do the advanced rebuild. I know Entourage makes a copy, but in rare circumstances the backup can be corrupted. Having the duplicate is just an extra precautionary measure.
