The advanced rebuild worked. I rebooted and all was okay. Today, however, 
entourage is acting like a brand new program. The startup assistant launched. 
What do I do know?

Probably not. I would make a duplicate of your Identity then do the advanced
rebuild. I know Entourage makes a copy, but in rare circumstances the backup
can be corrupted. Having the duplicate is just an extra precautionary
measure. Knowing you can revert to what you have now should make you feel
more confident in running the rebuild.
