Not sure what CS 2 has to do with account credentials, or why either would
affect Entourage...

Are you using the rest of Entourage?  Are your emails, calendar, contacts
still there and just the Notes missing?

Either way, you might try rebuilding the database, but it sounds like you
probably need to recover from backup.  And yes, it's the file called
"database" in your Microsoft User Data folder.
