I've heard from a handful of users where this happened immediately
*after* a cache flush or db rebuild so I thought it was related to
sync errors.
Some additional data points:
- We have an Exchange 2003 Server so all users have Exchange accts
- First heard about this around the time we rolled out the 11.3.5 update
