I do (1) most of the time, (3) when it's useful (for verticals, mostly), and (2)
never (I don't use ECF). I find that letting the camera choose a focusing point
is too slow (even if it picks a good one), and in any case I often don't focus
on something that falls directly under a focusing point. My method is to [1] get
an exposure and hit AE lock (when necessary); [2] focus; and [3] frame. Of
course, often [1] and [2] can be combined into a single operation.

I don't find this method to be a hinderance, possibly because it duplicates the
basic technique I've been using since my FTb days. With the kinds of subjects I
tend to shoot, I usually focus in order to achieve a desired DOF, so in many
instances I don't focus directly on the main subject anyway. Letting the camera
select a focusing point is useless in those situations. And since I habitually
allow my eye to roam around the perimeter of the frame right up to the moment I
release the shutter (since often the most important elements of a photograph are
those that are excluded), ECF isn't much use to me either (and I find it too
slow as well).
