When you buy the camera body you receive a form to fill in and return to
Canon. Your free item should reach you within 28 days (though I got the x1.4
extender I went for in two weeks). Unfortunately I've lost the other bit of
the form so can't tell you what other conditions there are.

Your choice, but in that situation I'd sooner ask the friend to be sure!

I bought mine at York Cameras in London. Excellent, knowledgeable and
friendly staff. Got a very good deal on the Nikon gear I traded in too.
