I am especially appreciative of Terje's response below, and others that
responded to me off-list.

Indeed, Panorama Tools is quite capable of fixing almost all of my problems
shooting with a 16mm lens with all of my subjects just 7 feet away.  I was
impressed on how it can fix both the pin-cushioning problem and the light
fall-off problem.

One of the best features of Panorama Tools is that it is Freeware - with
amazing capability!

I had another suggestion to use a product called LensDoc, another PS plugin
sold by Andromeda ( http://www.andromeda.com/ ).  This product looked pretty
capable also.  It's biggest plus is that the user interface is pretty good.
It's minuses, as compared to Panorama Tools, are the it's not free ($89),
and that it doesn't have any capabilities for radial light fall-off
adjustment.

It's also interesting to note the different architecture of the two
products.  Panorama Tools works exclusively from mathematical equations.
LensDoc works from empirical values, measured for many lenses and cameras,
that are stored in a database.  In fact, you can custom profile your own
lens with LensDoc.  I don't have an opinion as to which architecture
approach is better - it was just interesting to note the very different
approach to solving the same problem.
