The 1DmII comes with an NC-E2 as standard - charges and discharges/
refreshes 1D batteries. You can hook up 2 batteries at a time.

The mkII's batteries are NiMH, not Lithium Ions, so I would be careful
about adopting any 'topping up' techniques that you may have used for the
BP-511s . My advice is to get a second battery and rotate them, as I did.
I got one from an eBay seller, brand new in box for 80 bucks I think (I'm
in the UK and they are nearly 100 GBP new here). The batteries are very
good and the camera seems to last a very long time on a charge. Obviously
less with constant IS lens usage and chimping ;-)

You get a FireWire cable so you can connect up to the Mac or PC. This is
necessary to enable Personal Functions, though Custom Functions are
tweaked on the back of the camera. I use card readers so apart from the
odd tweak to a PF, never hook up to the Mac.

I assume you mean a main battery as a backup? Yes I would. The camera
contains a 'backup battery' for time/date etc but that should last quite
a few years IIRC.

You mean the electronic remote release RS-80N3 - yes that works with the
1D. I have one.

Nice one. I have a 420EX but tend only to use flash for social
gatherings/party snaps etc. I have just bought a ringlight as I want to
get into macro shooting again.

One thing that is not included that used to be with the 1D is a wrist
strap E1 - it's a personal thing but I wouldn't be without one. I do
street snaps in stealth mode occasionally and walking about with a 1D and
14 f/2.8 swinging about, and shooting from the hip, means the camera can
easily be knocked out of my hand. The strap prevents this.

Think about getting a 3rd party camera strap - the one supplied screams
CANON DIGITAL in neon lettering and invites unwanted attention. Some here
have mentioned removable straps (see archives). I designed and made my
own system years ago.

If you're not short of cash (bit of an oxymoron when buying a 1D eh?)
then consider an Angle Finder C - I use it for macro and landscape work.

Or some small accessories like a 70-200 L IS :-)

I had a D60 for a couple of years previously, and Pentax manual focus on
film before that. I can honestly say that the quality level you will move
up through from the 10D to the 1D is pretty remarkable. The camera is
absolutely amazing and it's ability to be customised is excellent. It's
got to last me a long time, and I have no doubts it will.
