There are two general cases where my 580 fails me. The first is low batteries. If it isn't ready to go it wil fire anyway, and it might not work out. The second is that I don't have the flash lock screwed down tight enough and my flash has wiggled a little bit.
Having the battery pack works well with the first problem, as does using brand new Energizer Titanium E2 Lithium batteries.
