Julian, I read about 'Transillumination' and it's the principle I was instictively trying to achieve by placing the flash against the skin. Googleling on it I found medical equipment to detect veins! Why they don't use that in the hospital? (instead of playing with the needle inside the skin and leaving a black afterwards?)
