In this case, it seems likely that your model is reacting to the pre-flash.
However, I've had the same problem, twice, using studio flash.
It transpired that these people were reacting to the sound of the camera,
i.e. the shutter charging and mirror raising. One person was fine with an
EOS-1n, but had 90% eyes closed/partially closed with an RZ!
Just a "maybe?".
