I just finished the conversion of my C to a D, and according to my IA, (a very experienced, by the book old-timer!) as long as the conversion is specifically allowed by the Type Certificate, the only required paperwork is a logbook entry detailing the conversion.  As far as changing to the E style split elevator, that would require a 337.

Lynn,

Is it necessary to do a form 337 and get FAA approval for the items listed in Aircraft Specification A-787 for conversion of 415-C to 415-D?

What paperwork is *required* for the conversion?
