Ed and Wayne still questioned the use of NASA form for a TFR bust. I called AOPA legal counsel and discussed this. He said there are several cases in which NASA form was used after a TFR bust and FAA has honored the NASA form. The following link is is another article on this issue and paragraph #5 addresses the TFR issue if you don't want to read the whole article (which you should).
NASA form does cover FAR violations UNLESS they are intentional. e.g. low flying over a neighborhood is considered intentional. If I say anymore, it will be saying more than I know. I've never used the NASA form (knock on wood) but I have always carried the form and an addressed envelope in my plane (I also keep a copy of the article in AOPA magazine with the form).
