Well, first of all, the pedal itself is mounted upside down.  Second, the pedal 
I-beam cross sectioned shaft is also twisted, thus twisting the bolt.  He needs 
to replace that part.  It is an aluminum casting, and he should be able to get 
one from Vernon Gregory.
