I've used iPhoto4 and iPhoto5 fairly heavily, and I'm still looking
for a good solution for managing photos that I don't necessarily want
to make public.

I'm starting to use my flickr account more heavily for managing
photos, although I'm still not dumping every photo I take into it
(there are some flickr accounts with 10,000+ photos) flickr does allow
you to decide whether photos are public, friends and family or
private.

iPhoto5 works best if you can just leave it running, it likes to
reindex itself on start which can take as much as several minutes.

Gphoto works OK but is slow, I've been looking at a couple of
solutions for indexing photos in a postgres database (including
thumbnails as blobs) and leaving the photos themselves in a directory
tree in the filesystem.
