We did this in Ex5.5 but I don't recall if we did it via the built-in monitoring or a monitoring workstation running PerfMon and Blat. Either way, it was free and alerted us when the queue began to fill-up. If it was the later we just set a queue length point in PerfMon whereby an action (blat) would send email to us both internally and externally. Obviously, the external one was only helpful if someone was on-call and realized when they received it (via pager back then) that the time stamp was an hour or two old by the time they got it. They could then VPN in to their workstation and look at the MTA queues from there.
