We did this in Ex5.5 but I don't recall if we did it via the built-in
monitoring or a monitoring workstation running PerfMon and Blat. Either
way, it was free and alerted us when the queue began to fill-up. If it
was the later we just set a queue length point in PerfMon whereby an
action (blat) would send email to us both internally and externally.
Obviously, the external one was only helpful if someone was on-call and
realized when they received it (via pager back then) that the time stamp
was an hour or two old by the time they got it. They could then VPN in
to their workstation and look at the MTA queues from there.

Btw, if you find it is not NDRs but actual SPAM in your outbound queue
it would lead me to wonder if you are being used as a relay.

We are running Exchange 5.5, on occasion (like just now) our outbound
queue fills with spam. We just had 5500 spam emails (<>) no originator
in the queue.  AS a result, real email was not going out (or was VERY
delayed),. Is there any way we can be alerted of this instead of having
to manually check? Any advice to prevent it ot mitigate it. TIA

Also, why would there be spam in the outbound queue?
