This problem turned out to be caused by the NAI antivirus software. I had disabled it as part of troubleshooting. but I did not stop the services so it continued to cause the problem with only inbound messages with attachments.

Where did you get to know about the NDR? from the admin account? Can send us the NDR message for further forensic?
