Deersoft, a company founded by some of the SpamAssassin developers, was acquired by NAI/McAfee a few years ago. They had developed a version of SpamAssassin that sat on top of MS Exchange. This has been folded into the McAfee product line. Their SpamKiller product is designed for the desktop, and is not based on the SpamAssassin engine. From what I understand, the server level product is very good, and integrates well with Exchange.
You can still use the open-source version of SpamAssassin free of charge, but you'll need to load it onto a mail gateway box, preferably a UNIX derivative. Or, as someone else mentioned, you could buy a Baracudda box, which is essential a pre-built Linux box running SpamAssassin. It's pretty cheap, too.
