Hello: If you decide not to use a VPN you could use a proxy server in a DMZ to just proxy the connections for the specific ports from the Windows Messenger client to the LCS server. This provides some degree of security, depending on the firewall/proxy technology that you use.
SIP, the protocol used for the communications, allows a client to connect to the server using port 5060 (or 5061 if you are using an SSL/TLS connection--you may also use 443). Note that an ephemeral port (>1024) is used for the backlink. Also note that audio/video communications are conducted out-of-band from the SIP session and not within the SIP 5060 connection.
