You can put the "Display Name" in whatever format you want in AD during account creation. Unfortunately, without a schema update it doesn't happen automatically.
For example, our organization required a policy that the Exchange Display name for all of our accounts be in the form "Lastname, Firstname (DIVISIONNAME)". Exchange 5.5 allowed us to automate that. In AD we simply edit the "Display Name" of the field to display the same on all new accounts.
