I'm not sure if this helps but try article 828731. We are running exchange 2000 
on a win 2k server in a 2003 AD. After the update we could receive e-mail but 
could not send any to the outside world. I had to call Microsoft support 
because we could not figure out what was causing this. 

The microsoft tech had me run workaround #2. Then stop and start dns on your 
dc. You have to run this on all your dns servers. On the exchange side you have 
to flush then register dns. 

I am not sure if this will work  on w2k3 and an e2k3.
