I generally recommend not to use the External DNS option on the VS. It seems to cause more problems than it fixes.

But still, set your Exch server to use your ISP's DNS and test it. I have seen this on Exch 2000 that is using a 2000 server for DNS. It 'should' send to the A record if there is no MX record, but it looks like 2000 DNS and Exch 2000 working together never make that happen.
