External or internal emails? Which one is the bridgehead server? From the problematic server itself do a telnet test.
I'd suggest to disable all anti-virus and ensure that file level AV's exclusions are set. Then stop SMTP and rename the vsi 1 folder, restart smtp. Check NCSA log for further information.
